Clinical decision support for ophthalmic surgery
CER-AI
Preoperative evaluation for refractive surgery and IOL implantation
CER-AI is a clinical decision support platform for preoperative corneal ectasia risk screening and refractive surgery evaluation. It analyzes uploaded Pentacam images to support ophthalmologists assessing candidates for LASIK, PRK, and SMILE.
For corneal refractive surgery, CER-AI supports preoperative screening, corneal ectasia risk assessment and separate tissue-safety analysis before LASIK, PRK or SMILE. For refractive lens surgery and cataract surgery, it supports IOL category suitability assessment for monofocal, EDOF and multifocal pathways, lens selection, spherical IOL calculation and toric planning.
Preoperative risk screening is essential for patient safety before laser vision correction (refractive surgery).
CER-AI brings together four literature-informed assessments: Randleman ERSS, CER-AI-adapted NICE, Pentacam Final BAD-D and PS3. Each is evaluated independently, and its score or result is presented transparently to the surgeon.
When screening and separate tissue-safety checks permit treatment, the application performs procedure-specific planning calculations for LASIK, PRK or SMILE according to the surgeon’s chosen procedure and parameters.
CER-AI: transparent assessment, practical workflow, accessible online
CER-AI is not a black box: it presents Randleman ERSS, CER-AI-adapted NICE, Pentacam Final BAD-D and PS3 separately, with the available inputs, assessment criteria and reasons visible to the surgeon. These four literature-informed pathways are not blended into a hidden proprietary score. Bringing them together in one report helps the surgeon review the findings efficiently and trace how each result was reached.
The IOL module brings spherical lens-power calculation and, when needed, toric planning into the same workflow. After reviewing the measurements and selecting a lens, the surgeon starts both with one calculation action. Supported reviewed data can also be transferred to ESCRS with one click for an external calculation and comparison, reducing repeated data entry in a busy surgical clinic.
CER-AI is a web application, not software embedded in an imaging device. Authorized surgeons can access it online when needed without purchasing a device specifically to run CER-AI. Analysis still requires supported imaging and biometry reports. Access is currently free for approved physicians; a low-cost subscription is planned to keep the platform accessible.
In clinical use tested by Dr. Hüseyin Cengiz, an assessment taking approximately 30 minutes manually was presented as a report in approximately 2–3 minutes with CER-AI. Timing may vary with image quality, missing data and the case.
An integrated platform for refractive laser and lens surgery
CER-AI brings photo-based measurement reading, refractive laser surgery assessment and refractive lens surgery planning into one web platform. It reads supported Pentacam report images, evaluates corneal ectasia risk and procedure-specific safety for LASIK, PRK and SMILE, and presents findings and planning options for the surgeon. For lens surgery, Pentacam findings and the required biometry support lens-category recommendations and surgeon-selected lens planning.
Once the required measurements have been reviewed and the lens selected, one calculation action starts spherical IOL power calculation and, when indicated, toric planning. A separate one-click ESCRS transfer opens the external calculator with supported reviewed data, allowing the surgeon to complete the calculation and obtain its result report there. ESCRS availability, supported lens models and any required confirmations apply.
The required images depend on the module and eye being evaluated; a single Pentacam photograph does not provide all measurements needed for every assessment or IOL power calculation. Required biometry, such as axial length, comes from a supported biometry report. Unreadable or missing fields require confirmation.
“To the best of my knowledge, CER-AI is unique in the world in bringing this complete refractive surgery workflow together in one platform.” — Hüseyin Cengiz, M.D., Founder and Clinical Lead

CER-AI 3.0: From report photos to IOL and toric planning
Photograph or upload supported Pentacam and IOLMaster biometry reports. CER-AI uses AI-assisted reading to extract measurements for surgeon review, assess lens-category suitability and calculate spherical IOL power with toric planning when indicated. From the same web workflow, the surgeon can transfer reviewed measurements and the selected lens to ESCRS, including Toric mode for supported toric lenses, without installing an application or retyping the same biometry.
Enter the patient name, age and biological sex, then upload the supported report photographs. CER-AI reads the measurements, recommends suitable lens categories and lets the surgeon choose the lens before calculation. Missing or unreadable information requires confirmation.
